From airplane crashes, to industrial disasters and medical error: When things go terribly wrong, why do we blame human beings instead of bad design?

Loved this episode! So many interesting (and devastating) stories of how bad design led to avoidable disasters. Can't wait to listen to more of this show.

@matresstester This one about UX and airplane crashes!!! The whole podcast is binge-worthy. There's one about the design of 💩 emoji.

This blew my mind it was so good. The image of that hot roomful of experienced engineers (successfully) trying to stop a core meltdown at 3 Mule Island is riveting. Science storytelling at its finest.

Thanks so much for the recommendation! Loved the episode. Have you seen the actual control room of 3 Mile Island @epekilis? I just searched for it and was shocked. I would hate to be the engineer there: http://three-mile-island.weebly.com/uploads/2/0/3/4/20345209/2163998_orig.jpg No wonder what happened happened.
